Permanent companions require that a few requisites be met in order for them to join the player's party. For a few NPCs, there is more than one possible way to have them join. If you want to have temporary companions join for parts of the DLC (very common in past DLC), you can do so freely. If you want it to be a solo DLC, it can be. Dead Money was my favorite DLC ever, and I think a large part of that was the developers being able to design the DLC around a pre-set starting point with no equipment or companions.
